About the Role:
We’re looking for a passionate Case Manager to work with diverse groups, including rough sleepers and hard-to-reach communities. In this role, you’ll assess physical health needs, guide individuals toward essential treatment, and connect them with local community services.

Your responsibilities will include:

  • Conducting risk and needs assessments.
  • Designing personalised, holistic care packages.
  • Providing flexible, tailored support to meet individual needs.
  • This is a frontline role where you’ll build meaningful relationships and drive positive change in the lives of others.

About You:

  • Experience in the substance misuse sector.
  • A degree in Psychology, Sociology, Social Work, or equivalent (e.g., Diploma in Adult Care).
  • A proactive, flexible, and adaptable approach to supporting diverse needs.
